This session does not cover the instruction on work at height or associated equipment. The session content focuses on the consequences of not following a safe system of work and the human factors of behaviour to working safely. The video is set in a mocked up coroner’s inquest scenario establishing the factors surrounding the death of a site operative who fell from height whilst working on a RC Frame platform. The coroner asks questions of three individuals – an inspector from the Health and Safety Executive; a site operative who was working close by and witnessed the accident; the site manager responsible for the planning and supervision of the works.
